Although practice patterns vary throughout the world, in the United States, the most common method of prophylaxis against endophthalmitis is still topical antibiotics. In other parts of the world, such as Europe, intracameral antibiotics are favored. In the facilities we serve and for the ones who choose to use the intracameral route, moxifloxacin is often the preferred drug, although there is no consensus on which drug is best.
The use of moxifloxacin in this manner is considered off-label and requires a bit of due diligence to ensure the facility is taking all the appropriate steps to improve patient safety and reduce risks. Since moxifloxacin solution for intracameral injection is not commercially available, are you preparing it, or obtaining it from a compounding pharmacy? Do they specialize in ophthalmic preparations? If you’re using a compounding pharmacy, have you done your homework to make sure you’re working with a facility that is state licensed, 503B registered with the FDA and in good standing?
